During investigation of this disease, the abdominopel- vic CT scan showed a well-circumscribed presacral mass approximately 7.5 cm in diameter on the right side of the pelvis (figure 1). He received five cycles of chemotherapy with cyclophosphamide, doxorubicin, vincristine and prednisone (CHOP). Reassessment showed resolution of the abdominal and mediastinal lymph node masses, but the pelvic mass did not change in appearance. Magnetic resonance imagery (MRI) showed a vividly enhancing ovoid right-sided pelvic mass (dimensions: 71 Â 32 mm), abutting the right sacrum and intimately associated with anterior sacral nerve roots, but with no bony invasion (figures 2A and B). At laparotomy, we discovered an encapsulated retroperitoneal tumor, located laterally to the right of the rectum and extending caudally from the sacrum. We excised it completely. The surgical specimen was an oval, elastic, hard tumor, measuring 75 Â 35 mm. Its cut surface was tan-yellow (figure 3). Histopathologic examination demonstrated abundant collagen fibers and Schwann cells admixed with mature ganglion cells (figure 4). The tumor was diagnosed as sacral ganglioneuroma.
