RESUMEN En el presente trabajo se realiza una actualización de los aspectos conceptuales y fisiopatológicos del síncope y la hipotensión ortostática, proponiéndose un enfoque de enfrentamiento diagnóstico y terapéutico que tiene como base una comprensión etiopatológica del síndrome que permita una estratificación de los pacientes según el riesgo relativo de sufrir en el futuro episodios de muerte súbita cardiovascular y/o episodios de morbimortalidad de cualquier tipo. Se hace énfasis en el comportamiento del síncope y la hipotensión ortostática en los pacientes de la tercera edad, particularizando de esta manera los algoritmos diagnóstico-terapéuticos en este grupo. ABSTRACT In this paper a conceptual update on pathophysiological aspects of syncope and orthostatic hypotension is presented. An approach to diagnostic and therapeutic confrontation based on an understanding of the syndrome etiopathological is proposed to allow stratification of patients according to the relative risk of suffering episodes of sudden cardiovascular death and/or episodes of mortality of any kind in future. Emphasis is done on the behavior of syncope and orthostatic hypotension in elderly patients, thus particularizing diagnostic and therapeutic algorithms in this group.
