Occupational safety and health is needed to be in place for a company to attain long/term viability. Today, occupational safety and health issues have gained more prominent attention from the public and media. Communicating occupational safety and health issues helps creating awareness and understanding on these issues. The data and observations are drawn from the logistics service providers in Malaysia. The focus of this study was to design and pilot a questionnaire to measure the logistics worker’s attitudes to occupational safety and health using the Decomposed Theory of Planned Behaviour (DTPB). This is intended for subsequent use in a large/scale survey, which in turn will inform the design of an occupational safety and health intervention programme. The early phases of a research are described: belief elaboration, questionnaire development and a pilot study. This pilot/study addresses prevention of human resources loss and materials damage throughout the logistics processes. A survey has been filled/up by logistics employees. The questionnaire was piloted to identify and assess the relative importance of the determinants of occupational safety and health performance. The study consists of the analysis of the present situation and recommendations for the improvements. The questionnaire proved to be reliable, and preliminary analysis provided strong support for the predictive power of the DTPB. The results obtained are fourfold: provide insights on various occupational safety and health programs practiced by the company; inspect the level of awareness on occupational safety and health among the employees; determine the methods and contents of effective implementation on occupational safety and health issues; and identify employees’ preferred channel in receiving information about occupational safety and health. The flow of information, co/operation between the various parties of the chain, and the training of employees have proven to be important in improving the safety of a logistic chain. Some key issues involved in the theory are also pointed, which may be of interest to researchers involved in the design of DTPB questionnaires. : Awareness, behavior, communication channel, employee, occupational safety and health.
