ABSTRACT We present a rare clinical case of distal penile gangrene in a patient with type 2 diabetes mellitus ater the implantation of malleable penile prosthesis. Necrosis of the glans penis, urethral meatus, dorsal penile shat, and corpora cavernosa was observed, and a distal penectomy was performed. Keywords: gangrene; penis; prosthesis INTRODUCTION Development of gangrene of the penis is a rare condition ater implanta- tion of penile prosthesis. Penile ischaemia and gangrene can be caused by penile prosthesis, long-standing diabetes mellitus, chronic renal failure or end-stage atherosclerosis. 1 he excellent collateral circulation of the perine- um and lower abdomen supplying the penis makes compromise of the blood supply almost impossible except under unusual conditions. 2 Since few cases have been reported in the urological literature, it is diicult to assess the rate of such a devastating complication ater the penile prosthesis implanta- tion. 3–6 We review the literature and present a case of distal penile gangrene fol- lowing implantation of malleable penile prosthesis.
