Injection Placentrex in the Management of Oral Submucous Fibrosis
Abstract
Oral submucous fibrosis, an incidious chronic precancerous condition of oral cavity mainly occurs in Indian
sub continent. The incidence varies from 0.2 to 0.5% in India with higher percentage in our place (Hyderabad)
due to increased habits of chewing gutka, eating chilies and hard roti. The exact etiology is still obscure, but
many factors have been thought. It manifests as, burning sensation and inability to eat. Various agents have
been tried in treatment but none is promising. In the present prospective study, an attempt has been made to
study the effect of placenta extract in the management of oral submucous fibrosis with no significant side effects.
It has no contra indications and can be used in early stages of oral submucous fibrosis with good results and
effect is long standing.
Keywords: Placenta extract, oral submucous fibrosis, management.
1. INTRODUCTION:
Many people are subjected to stress and tension in this century. Most of them fall into the tension
relieving habits like smoking, alcoholism, pan tobacco or betel nut chewing. These habits not only
relieve tension to some extent but also produce bad effects on human body apart from being addictive.
Oral submucous fibrosis (OSMF) is one of the conditions resulting from these tensions relieving
habits. Oral submucous fibrosis has been well-established in Indian medical literature since the time
of Sushruta. In the modern literature it was first described by Schwartz. In India. Joshi (1953) was the
first to describe the condition and he called the condition “submucous fibrosis of the palate and pillars
(Moss et al. 1968). Oral submucous fibrosis is an insidious chronic disease of unknown etiology
affecting the mucosa of any part of the oral cavity and occasionally extending to pharynx and
oesophagous and rarely to larynx. The condition is sometimes associated with juxt a inflammatory
reaction followed by a fibro elastic change of the lamina propria with epithelial atropy leading to
stiffness of the oral mucosa causing trismus and inability to eat (Pindborg et al. 1972)
OSMF is a silent ailment which is on rise in the younger age group the disease is more prevalent in
india and more in our part (Hyderabad-Karnataka). The prevalence rate (Ramanjaneyulu et al. 1980)
varies from 0.2 to 0.5 % in India with a higher percentage being found in the southern areas of the
country. Large number of cases occurs between the age of 20 and 40 years (Simposum et al. 1968).
Various etiological factors have been suggested like betel nut chewing, high chilly intake,
malnutrition, Vitamin imbalance, autoimmunity and genetic predisposition (Daftary et al. 1992;
Kacker 1975). But the exact etiology of oral submucous fibrosis has not been established till today.
The pathogenesis of oral submucous fibrosis lies in the continuous action of mild irritants. Many
studies has suggested that oral submucous fibrosis is a precancerous condition and it was first
mentioned by Pay master J.C. in 1956 (Daftary et al. 1992).The close association between betel nut
