ELSEVIER International Journal of Cardiology 51 (1995) 98-99 Isolated right ventricular infarction in a case of hypertrophic cardiomyopathy S.R. Mittal*, Dinesh Babu, Ranjan Roy Cardiology Division, Department of Medicine. J.L. N. Medical College, Ajmer, Rajasthan, India Received 10 April 1995; accepted 10 April 1995 Abstract A case of isolated right ventricular infarction in a young female with hypertrophic cardiomyopathy is reported. This case highlights the importance of detailed evaluation of right ventricle in cases with hypertrophic cardiomyopathy. Keywords: Myocardial infarction; Hypertrophic cardiomyopathy; Echocardiography 1. Introduction Isolated right ventricular infarction occurs usually in cases with right ventricular hypertrophy secondary to chronic obstructive lung diseaseor congenital heart disease [ 1,2]. We have observed a case of isolated infarction of right ventricular ante- rior wall in a case of hypertrophic cardio- myopathy. 2. Casereport A 25-year-old female was admitted with acute retrosternal discomfort and profuse sweating. Electrocardiogram revealed ST segment elevation with T wave inversion in chest leads V2 and V3. * Corresponding author X1/101, Brahampuri, Ajmer (Rajasthan), India, Pin. 305001. Echocardiography (Fig. 1) revealed gross hyper- trophy of interventricular septum and left ventric- ular lateral wall. All segments of the left ventricle were contracting normally. Examination of the right ventricle showedakinesia of the anterior wall with a normally contracting posterior wall. 3. Discussion Left ventricular infarctions are known in cases with hypertrophic cardiomyopathy [3]. To the best of our knowledge, there is no report of right ven- tricular infarction in a case of hypertrophic cardio- myopathy. Our patient did not undergo coronary angiography but, in view of her ageand sex, signif- icant coronary atherosclerosis is less likely [4]. Narrowing of intramural coronaries due to intimal-proliferation and/or hypertrophy has been observed at autopsy in young patients of hypertro- 0167-5273/95/$09.50 0 1995 Elsevier Science Ireland Ltd.
