25 Journal of Medical Sciences www.jms.yeniyuzyil.edu.tr Hematoloji Hastalarının Depresyon, Umutsuzluk ve Yaşam Kalitelerinin Değerlendirilmesi* Behice Belkıs ÇALIŞKAN Beykent Üniversitesi, Sağlık Bilimleri Yüksekokulu, Hemşirelik Bölümü, İstanbul, Türkiye Özet Amaç; Bu araştırma tanımlayıcı tipte bir araştırma olup, hema- toloji hastalarının depresyon, umutsuzluk ve yaşam kalitelerini değerlendirmek amacıyla gerçekleştirilmiştir. Yöntem; Araştırma Ankara ilinde bulunan Atatürk Eğitim ve Araştırma, Gazi Üniversitesi, Hacettepe Üniversitesi, Ankara Üniversitesi ve Ankara Numune Eğitim ve Araştırma Hastane- leri’nin hematoloji kliniklerinde tedavi görmekte olan 109 hasta ile Aralık 2014-Haziran 2015 tarihleri arasında yapılmıştır. Has- talara sosyo-demografik veri toplama formu, Beck Depresyon Ölçeği(BDÖ), Beck Umutsuzluk Ölçeği (BUÖ) ve Yaşam Kali- tesi Ölçeği (EORTC QLQ-C30) uygulanmıştır. Bulgu; Araştırmada hematoloji hastalarının %19.3’ü 18-25 yaş gurubunda ve %58.7’si lösemi tanısı ile izlendikleri saptanmıştır. Hematoloji hastalarının yaşları ile bedensel kaygılar alt ölçeği, gelecekle ,ilgili beklentiler alt ölçeği, motivasyon kaybı alt ölçeği, fonksiyonel durum alt ölçeği, BUÖ puan ortancaları ve EORTC QLQ-C30 genel ölçek puan ortancaları arasındaki fark istatistik- sel olarak anlamlı bulunmuştur. Katılımcıların eğitim durumları ile umutsuzluk ve suçluluk duygusu alt ölçeği; tedavi görülen kurum puan ortancası ile kendine yönelik olumsuz duygular alt ölçeği puan ortancaları arasındaki fark istatistiksel olarak an- lamlı bulunmuştur. BDÖ genel puan ortancası ile BDÖ alt ölçeği olan umutsuzluk arasında güçlü, pozitif yönlü ve doğrusal; BDÖ ile BUÖ genel puan ortancaları arasında orta, pozitif yönlü ve doğrusal bir ilişki olduğu bulunmuştur. Çalışmada kullanılan BUÖ ile EORTC QLQ-C30 genel puan ortancaları arasında po- zitif yönlü, bir ilişki olduğu bulunmuştur. Sonuç; Hematoloji hastalarının tedavi sürecinde depresyona yatkın oldukları, umutsuzluk yaşadıkları ve bu doğrultuda ise yaşam kalitelerinin olumsuz etkilendiği sonucuna ulaşılmıştır. Anahtar kelimeler: Hematolojik hastalıklar, depresyon, umutsuzluk, yaşam kalitesi Evaluation of Depression, Quality of Life and Hopelessness Levels of Hematologic Patients* Abstract Aim: This research is a descriptive study and it was conducted to evaluate the depression, hopelessness and quality of life of hema- tology patients. Method: The study was carried out between December 2014 and June 2015 with 109 patients being treated at hematology clinics of Ankara Atatürk Education and Research, Gazi University, Hacet- tepe University, Ankara University and Ankara Numune Educa- tion and Research Hospitals in Ankara. Socio-demographic data collection form, Beck Depression Inventory (BDI), Beck Hopeles- sness Scale (BDI) and Quality of Life Scale (EORTC QLQ-C30) were applied to the patients. Findings: In the study, 19.3% of the hematology patients were in the 18-25 age group and 58.7% of them has leukemia diagno- sis. The difference between hematology patients’ age and physical anxiety which is the BDI subscale, future expectations, motivati- on loss which are the BHS subscales, functional status which is the EORTC QLQ-C30 subscale, BDI score median and EORTC QLQ-C30 general scale score median were statistically significant. In addition, the difference between the participants' educational level and hopelessness and guilt sense subscale; the median sco- re of the institution receiving treatment and the negative emotion subscale score median were statistically significant. There is a strong, positive, linear relationship between the BDI general score and hopelessness which is the BDI subscale. There is a medium positive linear relationship between the BDI and the BHS overall score points. BHS andEORTC QLQ-C30 scales has a positive relationship between their general score averages. Conclusion: Hematology patients were found to be susceptible to depression, despair, and in this way the quality of life was adver- sely affected.
