A Patient with Anti-LGI1 Related Autoimmune Limbic
Encephalitis with Delayed Treatment

Anti-leucine-rich glioma inactivated- protein 1 limbic encephalitis (LGI1 LE) is a rare neuro-
logical disorder characterized by subacute progressive encephalopathy. There is limited data
on the efects of treatment timing and clinical outcomes.
In this case report, we present a patient with anti-LGI1 LE that had an early diagnosis but
delayed treatment and discuss the treatment outcomes. A 65-year-old woman was admitted
to our hospital’s neurology clinic 2 years ago with complaints of progressive forgetfulness
and behavioral changes. However, soon after the diagnosis of anti-LGI1 related autoimmune
limbic encephalitis, the patient refused treatment with intravenous immunoglobulin (IVIg).
Two years later, the patient came back to our clinic with slow worsening of her symptoms, and
consented to receive treatment this time. Following the treatment with IVIg, her symptoms
were dramatically improved. This case demonstrates that it is possible to achieve a good
clinical outcome in autoimmune limbic encephalitis patients even with delayed treatment.
