The coverage of exclusive breastfeeding in Purun Kecil Puskesmas in Mempawah District in 2014 was 86%, in 2015 it was 77%, and in 2016 continued to decline by 28%. The decline in the trend indicates the lack of lactation management practices and support from the mother’s family regarding exclusive breastfeeding. Family members, the husband and parents are considered as the parties most capable of giving inuence to the mother to maximize exclusive breastfeeding. Objective: To determine the relationship between breast care time, breastfeeding method, duration of breastfeeding, frequency of breastfeeding and family support for exclusive breastfeeding failure. Method: Analytical observation with cross sectional design. The population is mothers in the Madurese community who have 6-12 months babies in the working area of the Purun Kecil Puskesmas in Mempawah District. The total sample was 162 people using the total sampling technique. Data analysis used univariate analysis and bivariate analysis using the chi square test. Result: There is a relationship between breast care time (p-value = 0,000), breastfeeding method (p-value = 0.010), frequency of breastfeeding (p-value = 0.034), family support (p-value= 0,000) to failure of exclusive breastfeeding. There is no relationship between duration of breastfeeding and exclusive breastfeeding failure (p- value = 0.444). Conclusion: Lactation management and family support affects exclusive breastfeeding failure of mothers in Madura community in Puskesmas Purun Kecil region. Keywords: Lactation Management, Family Support, Exclusive Breastfeeding Intisari Latar belakang: ASI merupakan sumber makanan tunggal untuk bayi sampai 6 bulan pertama kehidupannya. Cakupan ASI eksklusif di Puskesmas Purun Kecil Kabupaten Mempawah pada tahun 2014 sebesar 86%, tahun 2015 sebesar 77%, dan tahun 2016 terus menurun sebesar 28%. Terjadinya penurunan tren tersebut menandakan bahwa kurangnya praktik manajemen laktasi dan dukungan dari keluarga ibu mengenai pemberian ASI eksklusif. Anggota keluarga yaitu suami dan orang tua dianggap sebagai pihak yang paling mampu memberikan pengaruh kepada ibu untuk memaksimalkan pemberian ASI eksklusif.
