This paper explores the feasibility of the inclusion of information and communications technologies for helping and localizing risk and early discharge patients, and suggests innovative actions in the area of E- Health services. The system will be applied to patients with cardiovascular or Chronic Obstructive Pulmonarry Diseases (COPD) as well as to ambulatory surgery patients. The proposed system will allow to transmit the patient’s location and some information about their illness to the Hospital or care centre. Keywords: Ubiquitous computing, ambient intelligence, telecare, mobile location. 1 Introduction The model for delivery of health services in our country is organized into a hierarchical structure of health facilities centres, where professionals provide services to their patients. The health professional contact with the patient begins in primary care centres, while the specialized care and management of the disease are performed in hospitals of first, second and third level, according to the level of expertise in the treatment and patient care. In the hospital environment, given the high volume of activity and mobility, both medical and nurses need technological tools that facilitate access to patient medical information and aid them to get clinical decision, with a new form of interaction necessary among professionals, most natural and close to the user, in other words embedded in the working environment.
