203 A functional and aesthetic solution for saddle nose deformity: the use of the inferior turbinate bone Semer burun tamirinde alt konka kemi¤inin kullan›m›: Fonksiyonel ve estetik bir çözüm Orhan ÖZTURAN, M.D., 1 Tamer ERDEM, M.D., 1 Murat Cem M‹MAN, M.D., 1 Rezzan ERGUVAN, M.D. 2 The use of a new autologous material, the inferior turbinate bone, for nasal augmentation is presented together with surgical treatment of a 24-year-old male patient with moderately severe saddle nose deformity. In the postoperative period, no complications were observed. Photographs and three-dimensional com- puted tomography views obtained 13 months after the operation showed that the reconstruction area was highly free of postoperative resorption. The patient’s complaints disappeared, and he was satisfied with functional and cosmetic results. Moreover, a histologic evaluation which was made to assess the depth of the glandular component showed that the inferior turbinate bone could be used over the nasal dorsum in a smoother shape, retaining its overlying soft tissue. Bu yaz›da, hafif ve orta derecede bozukluk gösteren semer burunlar›n tamirinde baflvurulabilecek yeni bir materyal olarak alt konka kemi¤inin kullan›m›, orta de- recede deformitesi olan 24 yafl›ndaki erkek hastan›n tedavisiyle birlikte sunuldu. Ameliyat sonras› izleminde hastada komplilkasyon geliflmedi. Ameliyattan 13 ay sonra al›nan foto¤raf ve üç-boyutlu bilgisayarl› tomog- rafi görüntülerinde, rekonstrüksiyon alan›n›n rezorbe olmad›¤› görüldü. Ameliyat öncesindeki flikayetleri ta- mamen geçen hasta, fonksiyonel ve kozmetik sonuç- lardan memnun oldu¤unu belirtti. Ayr›ca, alt konka ke- mi¤inin yumuflak dokusunda bulunan glandüler yap›la- r›n derinli¤ini ortaya koymak amac›yla yap›lan histolojik çal›flmada, kemi¤in k›smen soyularak daha düzgün fle- killi olarak burun s›rt›nda kullan›labilece¤i görüldü. Saddle nose reconstruction represents a signifi- cant challenge for the surgeons. Since many more demands are received for reduction rhinoplasty, aug- mentation procedures appear less in the literature. The most common cause of saddle nose is surgi- cal or accidental trauma. The former is mainly asso- ciated with excessive submucous resection of the septal cartilage or reduction of the nose in rhino- plastic surgery. The occurrence of saddle nose defor- mity following submucous resection of the septal cartilage ranges from 0% to 2.6%. [1] Other rare dis- eases that cause distortion of the nasal bone and car- tilages include Wegener’s granulomatosis, relapsing polychondritis, leprosy, and syphilis.
