63 " ACTA GASTROENTEROL LATINOAM - MARZO 2009;VOL 39:Nº1 Resumen La insuficiencia intestinal continúa siendo una causa de morbilidad y mortalidad en niños y adultos. En la ac- tualidad todo paciente portador de insuficiencia intesti- nal debe ser evaluado por un equipo multidisciplinario capaz de optimizar el soporte parenteral, determinar po- tencialidad de rehabilitación o indicar el trasplante. El Trasplante Intestinal (TxI) ha sido el último de los tras- plantes de órganos abdominales en desarrollarse. Final- mente, las indicaciones y los procedimientos se han estan- darizado; nuevas drogas inmunosupresoras y la experien- cia en el manejo de estos pacientes concentrada en un número limitado de centros ha permitido reducir las complicaciones y mejorar la sobrevida temprana y aleja- da. Cuarenta años después de la experiencia inicial el TxI puede ser ofrecido también en Latinoamérica. El ob- jetivo de la siguiente revisión es actualizar la situación del TxI enfatizando los progresos recientes y mostrando su evolución en el mundo y en nuestro continente, pre- tendiendo además brindar a nutricionistas, gastroente- rólogos y cirujanos generales los conocimientos básicos pa- ra entender la necesidad del manejo integral de los pa- cientes con insuficiencia intestinal. Review and description of its evolution in Latin America Summary Intestinal failure remains a cause of morbidity and mor- tality in children and adults. Currently intestinal failure patients must be evaluated by a multidisciplinary team in order to optimize parenteral support, to determine the chances for rehabilitation or the need for transplantation. Intestinal transplant has been the last abdominal organ to evolve. Indications and procedures have been refined and standardized; new immunosuppressive drugs and the experience of treating these patients in a limited number of centres have reduced complications and improved sur- vival. Forty years after the first attempt, intestinal trans- plant can now be successfully performed in Latin-Ameri- ca. The aims of this review are to update the current sta- tus of intestinal transplant, to highlight new develop- ments and the evolution in our continent and to provide the basic knowledge in the management of intestinal fai- lure patients for nutritionists, gastroenterologists and sur- geons.
