568 Abstract Background: Iatrogenic hallux varus is a rare deformity linked to bunion surgery at late adult age. Here reported is the first adolescent case of acquired hallux varus and medial dislocation of both sesamoid bones. Case report: The patient had had a surgical intervention under his first metatarsophalangeal joint when he was 10 years old. Correction of the deformity with a tendon transfer and medial capsular release alone – as was recommended for adults – was impossible in this adolescent, 8 years af- ter the index surgery. Excision of the contracted medial structures and repair of the lateral retinaculum of the fibu- lar sesamoid obtained a perfect correction of the dislo- cated sesamoid bones. Keywords Foot deformities · Hallux · Adolescent · Sesamoid bones · Postoperative complications Introduction Iatrogenic hallux varus deformity has been occasionally seen after hallux valgus surgery in adulthood [2, 3, 5, 7, 10, 11]. The addition of extensor hallucis longus tendon transfer to the medial soft tissue release has been recom- mended in most cases of hallux varus. However, not enough data is available about childhood iatrogenic hallux varus. Here we report an adolescent who had a neglected case of hallux varus with medial dislocation of both sesamoids af- ter surgery for a foreign body at the plantar side of his foot. Case report An 18-year-old man had difficulty wearing a shoe due to a deformity on his big toe. In his history, there had been a surgical intervention, in another hospital when he was 10 years old, for a foreign body under his first and second metatarsal heads. The deformity had formed during the 6 months after that surgery. Clinical examination revealed hallux varus, clawing of his big toe, hyperextension of the metatarsophalangeal joint, bowstring of the extensor hal- lucis longus tendon and a surgical, longitudinal scar under the first and second metatarsal heads. Plain radiographs and magnetic resonance imaging demonstrated medial dislocation of both sesamoids (Figs. 1, 2).
